There is a TSB for pulling/drifting to the right.
ST00305 I think it's called. ECU needed to be swopped.

I've been fighting a similar problem with my 06 400h.
It's been on 8-10 alignments but still behaves bad and my shop have been in contact with Lexus corp central as well Lexus Europe but they have no clue.

To get my RX400h to follow a straight line and a straight road especially crowned roads. I need to keep the steering wheel turned left by force to "3 minutes to twelve". It I let go of the wheel it will snap back to twelve at once and the car will drift/pull fast to the right.

I'm not sure if this is due to way to much caster or if it's something else but the 07 models I've tested are absolute exact and the steering wheel can be let go of and it will keep the exact position yu left it in no matter how the road look like. Where I with mine on the same road need to keep a firm grip of the wheel turned left to get the car to keep going straight.
Though the 07 have a slightly different EPS wich might be a factor.

It this the same thing you get?
Perhaps we need to start a new thread for that only?

I have an appointment on Monday. In the mean time I looked at the maintenance records online and see the car has already had four alignments done by the dealer. The last one was just a few hundred miles ago. I'm starting to wonder if this isn't endemic with this model. Since I have it in writing that they will fix to my approval, I will not settle for anything less than a car that drives straight down the road. At this point the car pulls hard to the right.

Is this an issue with this model?

Default Cause of Issue?

What did the issue end up being? Having the exact same problem with me 2008 Lexus RX 400H, dealership says it is aligned...

Two things that helped us a lot.

New tires. Better ones with higher load capacity.
Pirelli Scorpion Verde 235/55 R18 100W

And increased camber on the rear.
Our dealership swiped some bolts to more angled ones which made great difference.

but the new tires did a lot. It’s not all gone but I’m happy enough.

I'm interested to hear that they swapped some bolts. Were they camber bolts that go where the strut connects to the knuckle, where they OEM?

I'm not sure about rear, but for front struts there are OEMs:
