Update - multiple hardware and vehicle trouble shooting sessions narrowed the problem to software. Remedy is ultimately updated S/W from Japan. We opted to stop waiting (after a couple months with a loaner) and Lexus bought the RX back.
Ours was a very early VIN. Ultimately, I have confidence in the Toyota engineering team and expect ours was the exception.
Lexus handled the buyback professionally, as you would expect. Newport Beach kept us in an RX loaner through out and did not complain either. So, it’s a one-off and it was handled as best able. So, we have a replacement RX on order, this time an RX350h.

The turbos on these cars are vacuumed controlled for the wastegate. It’s actually more reliable and a conservative approach for long term reliability. I would bet that the fitting came loose.
