Finally I’ve done it myself.
this is after fixing as original.
after adding more LEDs
Separate the assembly by heated air ( it takes about 40 mints), and checked the wiring and there was a disconnected wires. Just soldering the pole again and that’s it.
It seems if 1 LED Bulb is missing, the whole row wilt run off, the soldering can be broke due to opening and closing the trunk.Anyway I have added extra bulbs to the whole assembly and it looks much more better.

Had this issue with mine after I bought it 3 leds out in right taillight. Bought from Carmax with warranty, but since found within 30 days covered fully by CarMax no deductible. They replaced the entire assembly.

the mod is awesome and should have looked like that in the first place! I can’t seem to find LEDs that would work. Could you let me know what brand and/or part/ part number for the LEDs so me and others can fix this problem

Lonreck appears to no longer be an active member. He has not posted since 2018. He failed to respond to a request in 2019 for more info on his light fix and it appears he will not be aware of or respond to your recent inquiry.
You may want to check the lighting forum or try to compare the LEDs visually to other available LEDs as similar ones are certainly used on many other Toyota and Lexus vehicles.
FYI when those burnt out on mine, I splurged on $600 to replace both tail lights on the trunk lid. I no longer have the old ones or would offer to ship them to you for parts.
Which leads to suggesting you look for parts donor tail lights on eBay. Maybe the price will be acceptable. It appears the term tail lamp gets more results than taillight, so search both ways.

thanks for the info! I’ll look into all that for sure. Appreciate the help

I had to fix the left LED tail light assembly on the HS250h that I inherited from my father. I needed to fix it to get a safety check to transfer the car to me in Ontario Canada.
I had to import the car to Canada from Florida, but that's a different story.
I took the assembly apart as per DW McCauley's instructions. Except that I pried the clear lens from the base, instead of heating it. I had to break the glue seal a bit at a time. It was tricky and I snapped a small piece of the clear lens off.
I found that the 6 LED are grouped into 2 groups of 3. It seems to take the 12v from the headlight voltage and run it in series over 3 LED on both sides. So if any LED in the group of 3 is burned out none of the 3 will work.
I took a AA battery pack I had for XMas lights and cut it off. So I had a 3 AA pack for 4.5 V. I could test each LED with that and found that I had only 2 good ones of the 6 total.
Then I drilled out the bad ones. I took a small bit and ground the head off of each rivet. Then I popped the old bad LED out and replaced it with a new one.
I shopped for specialty 1W flat mount high intensity red LED, but could only get 2. So I bought super cheap old fashioned LED (5 for a dollar!).
I soldered them directly to the board where I popped the old ones out.
The lens didn't fit as well over the older type LED, but it did fit in.
To test, I bought a cheap 3 pin connector. I bought a 9V battery snap connector and connected them together.
Then I could plug the 9V battery into the board and see all 6 LED light up.
I'll find out tomorrow if I pass the safety inspection, but it should work.
Total cost was under $10. I should have taken a pic. Maybe I will later.
Michael
