Happy to hear you had a simple solution just like mine. These cars are pretty bullet proof, can take a lot of abuse or neglect and can bounce back with just a bit of love! Happy motoring.

So an update on my issues. Had Lexus do an oil change but not a full flush, 4 litres in. Had them reconnect the hose that was off. The service advisor said it was the evap system and likely the cause of my p 174 code. Did some city driving and a 160k of highway driving and the result is no codes. The noise the pump was making in reverse is now gone. Transmission is shifting just fine, I suspect a low fluid level. So happy camper! Thanks for all who took the time to give me some guidance.
The pan volume on these is unusually small - less than 2qts. If they only drained the pan once, and then added 4L, you were almost 2.5qts low. I have seen this before (and even lower). Fortunately, the A760 is really robust.

Mine was the same. No idea how far it was driven with low oil. Since topping up it's been perfect.

Yeah the 760 - 61 transmissions seem pretty robust. The Lexus dude said they never see them in for anything but fluid changes. I mention my poor shifting transmission in my highlander and he guessed the year and was off by 1 - he guessed 2004, mine is 2005. The dude that tried to sell me a new one appeared clueless as I doubt they'd ever serviced one. Now time to turn my attention to fun things like a possible sway bar upgrade
