Default Steering rack bad???

Since day one I bought the car the car has had this problem. When driving only when the left front wheel goes over a pot hole the steering wheel breaks loose. The right front is fine. I have replaced the upper ball joints, lower ball joints, tie rods, and steering rack bushings. also when going really slow over a speed bump, the left front clicks(like a broken axle sound). i took it to lexus and they told me it was the lower ball joints before i had them replaced. please help..im taking it back there to se what else it would be..
my guess is the rack is worn...any sugestions?
