I have an '07 es350 with 278k that is throwing the P0300 code with all 6 cylinders showing up on the pending codes. I have replaced the coil packs and plugs about a month ago when the error first appeared. It now randomly comes back. I can drive it a day or two without issue, but the check engine and VSC lights are coming back. The #4 injector wire has been replaced as the wire insulation was damaged. No other codes so far. Has anyone seen this before?

 

Fuel starvation?

 

Maybe. I don't see any codes for fuel pump, but that doesn't mean it's not going bad. I thought possible injector clogged, but no specific cylinder is implicated. Coil pack on cylinder 5 was bad the first time the code appeared. Given the car's mileage, I just replaced them all with plugs.

 

The filter in the tank may be clogged.
You should take a look at freeze frame data.
