UPDATE: 2 years later and I think I found the culprit.

I believe it's a bad lug nut and/or stud in the front left hub.
Need to do more troubleshooting to figure out why the lug nut is not going all the way down to the rotor hat.
Cleaned the rotor and the wheel mating surfaces to make sure they're perfectly smooth. Also used a wire wheel on all the studs.
As a result of uneven torque on the left front wheel, the tire has become out of round and the front rotors have warped after just 2 years.

I will update with more details, once the lug nuts are fixed.

I could write a novel on front end suspension/vibration at this point......

Thank you, I can't put into words how euphoric I felt when I saw the lug nut not sitting flush with the rotor. It's like a light bulb that came on and gave me flashbacks that answered all previous theories.

p.s.: My first guess was rust, hence the text in the photo. However I believe it's actually stripped threads on either the lug nut or the struts or both.
Notice how small that gap is (less than a 1/4"). This is why it was so hard to see and detect with the wheel on.

i realize this is a two yo question but it is still worth answering. B/A is only a reboxer so their quality has a very wide and unpredictable range. You may get actual OE or pure junk. B/A developed an outstanding reputation in their early days because the vast majority of their parts were, in fact, OE. I once got a water pump which was OE with the Toyota p/n ground off the casting. You can call B/A and they will tell you COO but will not or can not tell you if it OE.
