I just bought a 2010 LS460 from a Lexus Dealer and was told that it was "Certified". Had 19K miles on the car and the service manual (which had no entrys in it) said the next service was at
20K miles.
My question is if the car was certified was the 20K mile service done?
Certified cars has nothing to do with early maintenance.
20k mile service is done at 20k miles.
Don't assume the dealer decided to do it early. You paying for service is still profitable to the dealership. Early "courtesy" maintenance is not profitable.

You can register online at lexus.com and see the service history of the car. If it was done at a dealership it will be there. If it was done somewhere else the original owner may have posted the service.
"Certified", in Lexus-speak, and as defined by most automakers who have Certified programs, means that the car has been previously-owned but has not been in a serious accident, has had any minor accident work repaired to factory-standards, meets strict age/mileage limits, has passed over a hundered visual and road-test inspections, has had a documented service history, has had any applicable worn/inoperable parts replaced, and has been detailed to new-car standards as much as possible. Unfortunately, from what I've seen, that is sometimes more of a reality on paper than in actual life....but, in general, Certified used cars are noticeably better than non-certified ones. As you would expect, though, in general, they also sell for somewhat more than non-certified ones, primarily because of the extra work/repairs/conditioning required for their preparation.
Don't forget the 100,000 mile power train warranty on a certified car. That has a significant monetary value, and Lexus has no deductible for any repairs.
