Floor Mat Safety Issue

The issue occured when I pushed
the pedal quickly all the way to the floor.
I believe the problem is the distance

the carpet when the pedal is pushed all the way

When the mat slides forward enough

Wow, how incredibly sad. I have never heard of anything like this. It makes me grateful my dealer only has the carpeted floor mats in their loaner cars. I'll put up with the dirty carpet.
It sounds like it hasn't been determined yet in this accident, but that seemed to be the issue in the original recall of the drivers side all-weather mat in the ES350.
In my car it was just with one mat, the all weather one.
I had a closer look at the all weather mat.
It has a rounded lip at the front edge.
It is then thin for about a quarter of an inch or so and then gets very thick.
I think having the thick area being under the pedal (when full depressed) results in the pedal getting stuck.

It's very simple. Don't use them both at once. Just do what I do. If the factory ones are fabric/carpeted ones or have custom factory logos on them, take them out and store them in a nice, dark dry place where they won't mildew or fade from the sun, and put in an inexpensive set of rubber or vinyl all-weather ones that are easier to clean (you can get them for as low as $20 at a place like Pep Boys, Advance Auto Parts, etc...). Then, if the rubber ones wear out, they can be replaced cheaply while the good, new, custom-made ones stay like new. When you are ready to sell or trade the car, simply put the original fabric ones back in.......they will be like new and (may?) add to the car's value.
i watched this video and im going man that sucks they died but if i knwo im going this fast and my accelerator is stuck im gonna do one of 2 things.
turn the car off......and try and stay outta peoples way or put it in neutral and let it rev till it blows the motor up or i slow down.
i mean i would hate to blow up my motor but i also would hate to die.... just my $0.02
