Its pretty easy to make your own. I only used 1 of the 2 plates that i bought (which were home made), but I wouldn't even know where to start looking for it.
I left everything intact and just slipped a plate between the egr pipe and the block + resistor mod. been happy boostin since and it really looks like my car still has egr.

90$ or w/e companies are charging for a 1/4 inch thick piece of aluminum is just RIDICULOUS. 400% over prices IMO. Draw the template go to a shop have them just make u it for what 20$? Not even... Have em just level one side (obviously not both cus ull use only one side to block it off. Get some gasket sealant from Kragen or Autozone and put it on! And ur Done!

on the little connector that plugs into the egr temperature probe looking thing.
you can leave the probe in place with its connector tied up somewhere, just plug the resistor into the car side of the connector.
I want to say that if you go to your local car store you can probably find a plate pretty similar and just drill out 2 holes. I have used quite a few gaskets and such they keep in stock for big block chevys and fords with a few adjustments here n there. like fitting a 3bolt 3" collector gasket to my 3" 2 bolt exhaust with a bit of shaving and aligning. not gonna pay 10 bucks for 1 3" JDM gasket.
