I thought D4S is some mode certain Toyota engines go into during "hot idle" using the port injectors to help clean the direct injectors. I personaly use the B12 or DuraLube severe system cleaner every 5k miles.

d4s is what toy calls it's own modern combo gdi system. in ultra-lean mode (under light load - exactly what is described itt and elsewhere) it needs the 'cloud' to form perfectly (oversimplifying ok). that puts additional requirements on the DIs (and they are super high pressure, high-voltage driven).
i _guess_ techron may do. i remember using some on an older car with some satisfying results

edit: wait, us ls4s don't light up the 'inj maint reqd' banner every 10k?

So car specs are as follows:
2009 LS460 AWD with 135k miles

I searched the general issue on the web removing lexus specific language and came across many articles pertaining to 02 sensors causing the exact issue i'm seeing in other cars. Considering the age and mileage is it unreasonable to assume they need to be replaced? I see almost no articles on the site about which 02 sensors go bad first. There are no error codes through OBDII scan

I did review other comments here and it appears as though TSB: L-SB-0013-12 AWD is what others have applied with varied results found here
