This happens to me all the time with my iPhone 6s while using Spotify. I feel like it has something to do with being plugged in to the usb and having bluetooth on as well.

I just had my Lexus in the shop yesterday and I mentioned the same Pandora tether error. They did a firmware update on my car and it is now FIXED. My SOS service was not working either and this was fixed by the update. I asked if the firmware update was something I could do and I was told NO, but I'm going to research further.

Lexus offered FW update via download but when I just checked the site it looks like nothing is available.

I was told by my service advisor that the FW update to fix the ether issue was an update only the dealer can do. It is not a multimedia update. Its worth bringing your car in to see if they say the same - it was under warranty for me.
