A video would really help!

Considering it doesn't happen all the time It's going to be nearly impossible to try to catch it on video. It happens at random timing but I can probably say more likely when the a/c is on ; not sure why.

It's part of the programming to help the car get up to temp quicker. If it's occurring outside of that time frame, I haven't experienced or heard of anyone else experiencing that issue. Honestly, the throttle body cleaning shouldn't be necessary with the port injection and direct injection we have on these cars. That was more of an issue on the IS250's since they didn't have the port injection and could experience build-up issues.
I too have this issue, but only in the winter time on cold start-ups and it stops when the car warms up. I took it to dealer last winter, but they found nothing wrong.

You need to upload it to a video/photo website and link it here. I use imageshack for photos and videos, or you can post it on youtube and share the link.

I'd suggest recording the sound, but also showing us your RPM. That would be very helpful. FYI my car (GS430) idles as low as 500rpm when warmed up with no ac running and in drive. In Neutral it goes up to 750/800 rpm. With AC on it goes to about 1000 or so.
