take the door panel off. then it's as easy as removing like 10 or 12 10mm bolts with a socket wrench. supporting the glass. removing the regulator, then installing the new regulator, and putting in all the screws.

Like deknick mentioned, it's pretty easy. I'm not a mechanic and have no experience and I replace my driver door while back without any problem. The only thing you need to be careful is to look at the original regulator and motor "position" before removing it. This way you won't have a problem of either the window sit to high or too low. I found out the hard way and end up removing it and took me a while before getting it in the right position. This is actually an easy DIY. Good luck.

I went to Fixed Right Lexus and one of the guys did the Read driver side door for me. He showed me how to do it. I took his tools and did the back window myself lol. It was a piece of cake. I just didnt know how i was going to get the window on the regulator. But it ws very simple.

Glad you got it done I was feeling the same when I first try to replace it and thought to myself how in the world I'm going to get this done, but once you remove the door panels, then the bolts, everything else seem easy since all the pieces are pretty much self explanatory why they are there. I guessed it's the genius of Lexus engineering. hehe
