What if you're in Kansas with new ES350 & they only have 89 Ethanol? Sorry if duplicate tried to post earlier

You fill it up and drive! You may lose a few horsepower over 93 octane. But your engine management system knows you have lower octane in the tank because of a knock sensor. Then it will lower your timing as needed to prevent any knock and consequent engine damage. Most drivers won't even notice a difference at all.

Around here, in IL, all grades of gas are 10% ethanol. All pumps have stickers with the info on them.

I've always heard that to get to 93 octane gas companies use alcohol which your engine doesn't burn very well...is that true?

Are you talking about E85 or just regular gas with +/-10% ethanol? If you're talking about E85, I would NO. That will cause fairly major problems with the fuel system. If it regular fuel with ethanol, then you don't have much of a choice. Almost all fuel now contains this corrosive junk. Whether the pump says it has it or not, it most likely does. Most states have a mandate now. You should be running startron enzyme fuel treatment through the fuel system on a very regular basis to counteract the horrible effects of the junk, then call your state government and tell them to take this stuff out of your gas!
