From what I gather so far it's a step further than the injector replacement I just did, so should be fine.

Is that a folded gasket? This appears to be a failure on a repair done? This did not come from the factory this way right?

Yep, folded gasket, causing vacuum leak.

Glad you finally found the vacuum leak. Those fuel trim percentages don't lie.
