The bang or clunk is from the bad trans and or engine mounts and the jerk is from the high idle due to the isc.

It's pretty simple really.

Engine/Tranny mount, and TPS setting, loosen the screws on the TPS and tap it down a little, should alleviate the problem.

ONLY MAKE MINUTE ADJUSTMENTS!! But yeah, my problem was mainly park to reverse.

I changed my tranny mount and flexible couplings

And the clunk and vibration went away for me.

I only have issues on 1st gear... it takes forever to shift to 2nd gear. Every gear does work but 1st.

I already changed the tranny mount a few months ago thinking it would solve the problem. But I'm not sure if we are talking about the same thing, it's not really a vibration but a huge jerk when you shift it from P to D at a cold start, but once you've warmed up the engine it doesn't do it anymore.

I believe your problem would be dirty oil and transmission oil filter/strainer. That's actually the real reason I'm going to do a flush on mine. But if it's gonna solve that jerking and clanking like what Bender25 said I'll update this thread as soon as I do it.

If the car is obd2 and tps is out of spec it will throw a cel and on my car I have done the trans fluid change at least 10x(DRAIN AND FILL) plus a filter and the problem is still the same.

Mine's OBD1. I don't think there's anything wrong on my TPS though, I've checked it when I did the leaky valve cover gasket. But I'm gonna replace it still in time.

I haven't received the ATF-IV I ordered from Sewell Lexus, that's about the only thing I'm waiting for. Because I only ordered 4quarts the first time and just decided to get the 12pack since its cheaper and it looks like I'm gonna need it.

Hey guys, I've just finished doing the transmission filter, transmission oil pan leak and ATF flush a few hours ago... And I'm sad to say that it didn't get rid of the jerk and clunk when you shift from P to D... I'm getting frustrated with this... I did a full flush even though the oil wasn't that dirty, and when I was tightening the drain plug it freaking cross-thread the plug, so I guess I'm gonna need to buy a new tranny oil pan and redo the whole thing in time, although I was able to tighten it enough that the oil isn't leaking (I hope)... But still the dreaded jerk and clunk isn't gone! I'm gonna try to mess around with that TPS right now...
