What over-the-air updates are you expecting?
The only over-the-air updates that I know of would be the Dynamic Navigation which is part of your Lexus Enform trials (remote start, safety connect, service connect, etc). Dynamic Navigation does cloud-based updates to the map data for 3 years. You can purchase an update to the Dynamic Navigation after 3 years for $350 plus labor at your Lexus dealership. After those 3 years, the map will revert to only the info it knew when it was new.
There will not be updates that add new features or revamp the look of media system. This is a Lexus, not a Tesla. Reliability over fancy tech.

On several vehicles when a problem is fixed in the firmware it is usually sent out OTA. In my Jaguar the fixes/updates are/were sent OTA. Map updates don't interest me much as the roads rarely change and if I want a POI I'll just use my phone. BTW I got rid of my Jag just because there were too many glitches in the vehicle.

I believe OTA updates on a Lexus are usually for the infotainment system not vehicle components. So things like App suite software running on infotainment (not your phone) and in 2020+ to things like Alexa (which runs as an app in infotainment) or bugs and fixes pertaining to crashing.

Vehicle component updates (such as service bulletins) have to be done via a dealer (at least historically) which requires their system that can patch into the CAN BUS.

There will not be any over the air updates that benefit anyone. https://securedp.lexus.com/download-app/downloads has multimedia updates that can be user installed with a USB drive, but these are just small patch updates to fix glitches. Lexus never retroactively adds features to their cars. Even pre-October 2019 production ES350's do NOT have Android Auto, even though the hardware is the same and a simple software update would add it.

So other than Lexus, where would one get (buy) the infotainment updates? Or is that only done at the Lexus Dealership?

I have a 2017 and the only OTA updates that I have seen were to "remove" features/apps rather than add them.

If you have "Destination Assist" as part of your Lexus Enform subscription package they push OTA navigation directions.
