Just an update:
#4 - Were were able to catch it in the act, so-to-speak, and recorded a video to show Lexus. They believed it to be the vacuum pump, which they replaced it on Monday. BTW - the pump was $1800!!, according to the service advisor. Thank goodness for warranty. Hopefully, this solved the rattle issue.
