Just thought I'd add a write up of my recent misfiring Rx episode.
Went out to collect some shopping a, week or so ago, on the return journey the car felt a bit lumpy and slowly got worse, the engine was obviously misfiring, however it was random, not continuous, anyway by the time I got home the car would hardly drive and the eml light was flashing.
Pulled onto the drive and turned it off. Went inside and got my code reader, tried it but no luck, no codes stored on the Ecu. Started the car again, it would run but was obviously very sick.
Because of the covid issue at present all Lexus main dealers were closed so I couldn't even call their service department for advice. In the end i managed to find a local garage that apparently specialised in Hybrids, they said they'd come and pick it up the following day, I told them they'd need to tow it as I wasn't happy with it being driven.
To cut a long story short, they couldn't collect it but said that'd come out and run a diagnostic. The guy turns up plugs on his Bluetooth reader and runs the check.
His diagnostics tell him that three coil packs had failed, 2 at the rear, one at the front. I was quite suspicious when he said that so stated searching forums and reading up on it, I've also read up on how to replace these and know how much of a pain the job is. OK, I agreed for them to take the car away after they have given me a quote. I also took a photo of his diagnostic screen.
While waiting for their quote I did a search of the forums to see what people's experiences were and I also checked out the diagnostics. The photo isn't very clear but most of the misfires are cylinder 2 and that's at the front, I also found out the healthy cylinders will also misfire if another fails.
Because the garage were so slow in responding I thought I'd order the parts and swap cylinder 2 coil and plug, the parts took a few days to arrive and I fitted them in 10 minutes.
The car started and ran perfectly not a hint of a misfire anywhere.
Moral of this story, don't alway believe what a garage tells you especially if you don't know them. I'd could have ended up with a bill running into many hundreds of £, fortunately it only cost me £100 to fix it myself.

You are right. Unfortunately these days; some garages/shops are more into making money then getting the customer a accurate diag. and proper quote etc. So yea it pays to do research on a garage and reviews for that shop/garage before spending any money; especially because ya just dont know sometimes who you can really trust; that and if its something u can do yerself then all the better to save money.
My issues with misfires were the coils or plugs. I had to change them. 2,4,6. Those are the easy ones. 1,3,5 Plugs are yet to be changed but it’s running good now.

The ecm gets a feedback pulse every time a coil fires sucessfully and adds a misfire count when it doesn't get the pulse.
But I have seen many misfires counted when coils were fine but car was runnig poorly due to other conditions and it makes me think the ecm also counts rpm drops over a certain spec between cylinders too. The crankshaft sensor voltage output changes with rpm so it knows a problem exists but it's not very accurate t detecting the cylinder that way.

My car was also very sick and scary. I was stupid to drive the car and it was miraculous there was not a mishap. It started giving me many codes like VSC, ABS, Road slippery hazard and before I could realise and decide to pull got a Hybrid System Failure and car stopped running and lost power.
My Code reader only gave me P0300 and P0354
P0300 - Random/Multiple Cylinder Misfire Detected
P0354 - Ignition Coil "D" Primary/Secondary Circuit.
The OBD reader is not telling me which cylinder is misfiring
