RX 300 shift issues

My 2002 RX 300 won't shift into 2nd gear. It also shifts slowly and rough into reverse.

What is the condition of the transmission fluid .. color/smell/level/?
Odo?
When and how the transmission was serviced last?
What is the idle speed when you try the reverse gear?

You mention 2nd gear .. how about selection "D" <first gear when moving from N?
