Blue tooth faulty
The Blue tooth on my 2016 ES 350 LEXUS seems not to be working. When I get a call I can hear clearly but the person on the other side does not hear when i talk.
Your ES has a phone mute function that is supposed to do exactly what you describe
for use during conference calls while your dogs in the back are barking... Watch the
phone screen during a call and find the mute button, tap it to unmute.
Second approach if that is not the solution, unpair the car from the phone and also delete
the phone from the car. When you repair the two be certain that the banner that appears
in your phone changes from "hand free" to "Lexus ES". If you tap the banner before the
Lexus ES appears, it will pair but incompletely which may include your symptom(s).

I had a similar issue on my RC350 (2015). There was a software update that was applied to the car at the delaership. Give that a go. Did wonders!
