Default Need Help, Transmission issue

Hey guys, I have a 1998 SC 300 and I'm having some issues with the transmission. Its the stock 4 speed auto. When I roll up to a stop sign but don't completely stop and then go back on the gas, the car is out of gear and all that hitting the gas does is bring up the revs, once i realize this i immediately take my foot off, and usually at some point while the rpm is going doing the car clunks into first gear and then i'm good. This doesn't happen every time, but it's happening more than I want it to. The car tends to do it at speeds under 15mph I have noticed but I notice it most when moving at around 5mph. So I was wondering if Anyone knows what this might be and how or if I can fix it. Let me know if there is anything else you guys need to know. Thanks for your time.

Check the fluid level with the engine warm, running and in park. If its low make sure you use the correct Toyota ATF. Sounds like your trans, may be going out.

Yea i only just replaced the fluid like 3k ago and the levels are good, that's what i was worried about, kinda knew in the back of my mind it might be going. That sucks
