Okay, I got the car back from the dealer and now I know why the procedure in the service manual didn't work. Apparently all the slots were filled up, so it wouldn't add any new ones. The solution was to go through the procedure to erase all the existing codes, then add back all the keys.
After I cleared the existing keys, the add key procedure worked fine.

How did you erase the keys? I have the instruction but it doesn't seem to work either! Thanks.
Dealer did it for him. Do you have the full instructions (i.e. the service manual), it would be in there (erasing instuctions plus programming instructions).

I know this is a few months old, but I am in the same boat. I have an '04 LS 430 and have tried these methods with a new key from Sewell. How do I erase a slot without going to the dealer. If there is know way what does the dealer charge? Thanks!
I tried and tried the procedure but couldn't get it to work. I took my key I bought online from sewell to the local lexus dealer and they did it successfully for $30. (I did not get confirmation that they needed to do the erase procedure)
I was there with my kids and they enjoyed the bagels and muffins so it was the first time I felt that I didn't get completely raped at the dealership. Just thought I'd share since this forum has helped me greatly in the past.
Well, I just bought a 2003 LS430 and got an additional key from Sewell. I tried the programming method they had on their website, with no luck. After reading these threads, I thought I'd give it one more try. I followed this method, but left out step 6 as was suggested in a later thread:
Alternate procedure to program transponder:
1. Make sure all doors and windows are closed.
2. Insert Master key into the ignition cylinder.
3. Cycle the ignition cylinder from On to OFF five times.
4. Open and close the driver side door 6 times.
5. Remove the Master Key and insert the new duplicate key. 6. TURN THE IGNITION CYLINDER ON. (DO NOT DO)
7. The new key will be registered after 60 seconds. The THEFT light on the dashboard will go out, indicating that programming has been completed.
Low and behold, it worked the first time.
Now I have to program the remote enttry. After three tries, I still haven't gotten it. Has anybody done it a different way than what Splogue spelled out?
StnMtnRX is right and step six is not in the current Factory Service Manual. One other tip I'll pass along. The FSM says you have 35 seconds to complete steps 3 and 4. I did it in 30 seconds and it didn't work. I decided to try being an ignition turning/door closing fool and did those steps in under 15 seconds and it worked. It appears the FSM uses metric seconds.
Also, you'll know it worked because the security light will come on solid. Then when you insert the new blank within 10 metric seconds, the light will start blinking and after 60 seconds, the light will stop blinking and go off and you know it is registered. Good luck.
