Neither the flat key (wallet) or number plate were provided when I purchased my LS400.

Both Master keys and Flat Key were provided when I purchased the LS430. But the Sub Key (valet) and the key number plate were not provided.

Is the Key Code that is stamped on the flat key case the same number that is on the key plate?

The key code stamped on the wallet key/flat key case is for cutting a new key to your car. It is the unique cut for your key

I'm not sure what the key plate is that you're referring to
