I've started to hear a tiny ticking rattle sound from around the B-pillar car/driver's door door when driving over irregular roads. The rest of the car so far is rock solid and quiet.

I had this same problem and thanks to joe Z t got fixed It was a e brake wire it had lots of tension since
It's a new car he told me to fully engage the e brake and leave it engage for ain't time I would say do it all the time over night do it for around three days and u should notice the difference it is gone by day two it worked like a charm!!!! Thanks to Joe Z saved me a trip to the dealership
^^^ I did the ebrake procedure the first days of ownership thanks to that post, but it's not that. It's a small squeak which I now think is from the grab handle. Will do more testing.
I'm confident it's the grab handle. Touching it, pulling on it, etc in any direction makes the tiny squeak and since it's right by my left ear, makes sense. Now to resolve this, I'm going to attempt this fix in the RX forum this weekend...
UPDATE: Troubleshooting process - Removed the grab handle....
UPDATE. In an attempt to isolate the issue, I removed the grab handle and so far today I have not heard the squeaks! Looks very promising.
However, I want to make sure it's not an environmental condition since today was warmer and more humid. Tomorrow will be cooler and drier. Will provide updates in the upcoming days.

semoquer/js 101. LOL it was almost the case! The only "problem" with the 10 years of Lexus vehicles I've owned is that because they're so quiet, you'll hear the slightest peep that you would otherwise never hear in another car. I can't imagine how bad the creaking was then on your five-O...
dojoman: It's the driver's side grab handle on the ceiling.
lf the squeaks are also consistently gone today, I'll attempt the fix and run the car another day and post the Pics/DIY/fix. Fingers crossed.
