I owned a 2007 RX400h so am not familiar with the IS250’s ventilation system, but I assume they’re conceptually the same. Each side has a motor-operated vent actuator that rotates to blow air through the various vents. That on the passenger side sits below the glove box just next to, and toward the front of, the console (toward the firewall).

The actuator might be jammed, damaged, etc. On my RX air blew out the top vent only.
