Default Jerk Under Acceleration

Hello, I have an IS220D. the car has been good to me but with one strange problem.

It seems to jerk under full throttle acceleration at specific rpm from 2 to 4 gear. The car would accelerate up to 2,500 rpm and then jerk once and then does it again at 3,100 rpm. What is even stranger is that it doesn't do it every time I fully accelerate but when it does, its those said rpms exactly. If I don't put my foot down and leave it lifted off the pedal ever so slightly but still accelerate hard, it doesn't jerk. The jerk is more like a sudden power cut for a millisecond and then the power is there again. It doesn't go into limp mode.

I've cleaned out the egr valve but no difference. I've tried injector cleaner but no difference.
