Wow, this is the EXACT same noise my 2016 450h is experience on some starts. Mine is a little louder and more violent, but after about ~20sec of idling the engine smooths out and it is fine. It is a CPO so I am taking it to the dealer. When you were having this issue, there were no CEL or warning displayed on your dash correct? Mine does not, just the audible engine noise and shaking.
Recently took the car in specifically for this problem. Previously looked at during two other scheduled services. Upon initial start of the car where: the car is turned on, there is a pause as the car initializes and finally the engine will start up. It is at this point where the engine stumbles badly. Sometimes it is very rough and there is some "clacking" noise coming I think from the engine bay. The car will stumble and proceed running rough as I drive away. The car will stumble upon moderate acceleration. At times during other parts of the day, the issue is less pronounced however the engine seems to miss while idling or slow maneuvers from a parking spot. Also, at normal operating temperatures, the engine does not turn off at a stop which I believe happens normally with the hybrid. Other times it does turn off. These symptoms will disappear and then reoccur without any clear common denominator.
Dealer again ran diagnostics without any revelations. They also contacted Lexus corporate offices for any additional information. No definitive cause only to say that it seems to be a fuel issue. I've only used top-tier premium grade fuel. Prior to bringing the car to service I ran two tanks of Chevron Techron fuel system cleaner to see if I could clear the situation up. Not so and therefore I took the car in for service.
I am try to stay with one brand of gas to see if I can narrow down any fuel problem but the stumbling continues.
Any comments on similar experiences is appreciated.
Did you have any check engine light on in the dash or any other indication that there was a problem, other than listening to the engine itself? My 2016 450h with 37K miles just started doing this.
I had my injectors replaced through tsb: L-SB-0045-12. My ‘17 feels much better and the stumbling seems to have stopped. But, as others have mentioned, it will likely come back.
I think I will use a fuel injector cleaners ever 5k.
Since the injectors were being removed (~45k), the dealership agreed to change the spark plugs for a nominal fee.
No CEL or codes will be triggered by this symptom................ a few months on and all is perfectly smooth at start up . Dont let anyone tell you different if you have this start behaviour . I suppose it woud cost quite a bit to get a garage to clean them . Luckily i can do it myself .

Injector cleaner every once in a while cant do any harm and may do some good !

If the car is with the dealership now, ask to have spark plugs replaced. I had them do it since the TSB covers half the labor anyway.
During this time that the car was running rough & then after the injectors were replaced, did you notice any difference in fuel mileage?
Sorry for the late response. After the injectors were replaced, there was no significant difference in mileage. Going through this topic, I see that others had similar issues with the same resolution. I now have around 67K miles without any issues.
