Rattle (Gear clatter?) after cold soak

Sounds very familiar. The fuel injectors foul up. Appears to be worse as it gets cooler, high moisture or car sits for a few hours. While under warranty, dealer replaced fuel injection system twice. Worked beautiful to resolve the problem for 30 - 40k miles before returning. Read our cars don't have a fuel filter, not sure if this is true. Could be the poor cleaning ability of a direct injection engine.
I've since ignored this rattled that came back around 100k miles. Car has 210k miles now, outside the morning rattle, runs fine, never had it not start or stall.

From what I understand the 3rd gen RX450h uses port injection and that Toyota's D-4S direction injection was introduced for the 4th gen 2016 RX450h. One of the reasons the 4th gen got a nice little power bump.
There were other models floating around at the time though that did sport the direct injection option, like the GS450h.

Looks like it’s my turn to have it.
2012 RX450 with 174k miles. The car care nut has a video taking the cam out and replacing the gear of a Rav4, but says the 2GR-FE engine suffers from this. Tho the rattle he demonstrates at the beginning of the video is different.
