Lexus 450h manufactured in March 2009 after maintenance or error: "ENGINE OVERHEAT". Please ask why?

Was the radiator flushed and not refilled properly? There should be fluid in the overflow, would check there.

 

Please write more details. Like what maintenance?

First suspect for cooling issues is coolant [is it full, is there air trapped]. If the coolant was drained during maintenance, did you put the vehicle in service mode to fill up and purge air].
Next is air flow? Is the radiator air flow good?
Is the thermostat working correctly?
Does the radiator fan run after the operating temperature is reached?

The EGR radiator is leaking (perforated). Resolved by replacing the exhaust EGR radiator. Thank you!
