Are you talking about something came loose inside the headrest itself?
There is a seam on the bottom side of the headrest between the two headrest posts. You can unclasp(two parts of the leather come together with a plastic connector, runs the length of the seam.) that and very carefully start to remove the headrest cover. Then fix whatever issue you have going on in the headrest.

I reupholstered the interior in my last car. All manufacturers basically use the same method to close the headrest cover, nobody actually stitches it shut, since the other way can be undone and faster to make. Also my business is all online so I work from home mostly.
**** i got a couple rattle to fix, that was one of my rattle list. heres the list of my rattle - front left and right speaker cover on the dash, coin holder tray, mybe the headliner have to look into this, and the headrest. ><
