Default GS300 Fuel trim

Hello,

I have problem with 2002 GS300(161kW). The car is running rich, high gas smell, bad engine performance etc.

Long-term fuel trim highway is constantly 30.5% both banks. There was a fault code regards to both O2 sensor heater circuit, I was able to change B1S1 o2 sensor but B2S1 was unreachable. The O2 sensors themselves are working, both showing mostly identical 1V high voltage(indicating rich?), from time to time they go close to 0V, so I guess there shouldn't be a problem. Why doesn't the ECU inject less fuel if the o2 sensor are sensing rich condition?

MAF seems to read fine, around 3-4g/s idle 800rpm and ~12g/s 2500rpm no load.

Thanks in advance for Your advice!
