That being the case I don't know why they would have to be replaced...

Seems you're on a merry-go-round and it's time to get off. I'm surprised that the Lexus dealer cannot pinpoint the issue. Since you're grasping at straws I'd throw in a can of Techron fuel additive and see if it helps. Cheap enough and good stuff... Use it in my 22 year old Corvette frequently. Wish you the best partner...
Thank you man, I took the spake plus and went to another shop and they told me nothing wrong with it. tomorrow morning i'm going to the shop and i will let them pay me back or suits them! good thing I have the spark plus with me and the diagnose report.

Maybe the laws wherever this car is are different (the location says Ohio). The OP said he sent a letter to Lexus, but never mentioned if there was a response. Personally, I'd call them. They'll likely say they can't control local dealers policies (they tried that on me, once), but they can. IT worries me that there was another car in the shop with similar symptoms. Is the fuel different?
I believe the warranty specifically excludes any cars sent overseas except the recall safety. fuel is fine but some Gas stations has bad Tanks with a lot of dust.

Lexus US replay to my email today and asked more information so they will pass it through to Toyota Motor Corporation in Japan. Wish me luck

that's the same thing I was thinking this changing spark plugs at 15,000 miles in clogged injectors makes no sense to me I just can't see that happening to this car at 15,000 miles also that mechanic sounds like he's just trying to make some money bad plugs that's ridiculous

The noise in the original post sounds like valve train noise. The 5th Gen ES has history of this, especially at start-up. There have been multiple threads regarding this. One poster fixed his noise by increasing his oil viscosity from 5W-30 to 10W-30. Here's the link:

Check the air filter to see if its clogged, or if a hose came undone. Where in the middle east are you?

As for the warranty you can use the warranty, aslong as you pay for the repairs and Lexus US will reimburse you. But like you mentioned dealers in ksa will not touch US cars. So you may have better luck in bahrain or uae if you needed dealer repair
