I believe with 2009 model year, Lexus began recommending an ATF flush every 60K if the car falls under Special Conditions, or severe service - towing and long idling.
An ATF flush should be based on level of fluid contamination, regardless of mileage. Best way for the DIY to determine level of contamination is a visual inspection of the fluid on white paper towel. The only exception is if the ATF is burnt and has metal flakes in it; if you change that ATF with these conditions, the trany will slip. Below is an excellent video by Chris Fix.

Lexus/toyota wont examine your ATF fluid unless you have issues shifting or rough shifts. It seems stupid, they usually like to make a quick buck doing all kinds of services you dont need.

i have 217k miles on my 07 ES
at 120k for you if you have not already done so
i would say get the surp belt changed have the spark plugs replaced have the coolant fluid changed trans fluid changed does not have to be flushed if you dont want to do that just have them drian out like as much as they can and then fill it back up its possible to do this because i had my dealership do it every 60k for me. also get a powersteering fluid service and if you use just only the cheapest fuel you can find that could be dirty have the fuel system services or at the very leased put a bottle of chevron techron from autozone into your gas tank have the air filter engine replaced and a oil change and you should be fine
this is al going ot be expensive but maintian it now for service or pay a hell of a lot more to fix it later for not doing it when recmended

ATF degrades as a function of time/miles at temperature. I believe it is irresponsible of Lexus to claim that the AT fluid is lifetime for anyone that wants to keep a functioning car for a long time beyond the initial warranty period.

Unless your old ATF looks like the new ATF (which is won't at 120k miles), do a couple of drain and fills.
When I recently did a drain and fill on the ES, both my wife and I noticed an improvement in shifting afterwards. The old fluid was noticeably darker than the WS fluid in the bottle, but not burned by any means. I previously had a couple of drain and fill done at my indy mechanic at 80k miles, and then I did it myself at 120k miles. Plan to do another drain and fill in about 20k miles.

All good points, BUT, dealer will not change the fluid. Not everyone has a good mechanic that can do the procedure correctly. It is not a simple procedure to do that you can go anywhere.

Some Toyota and Lexus dealers will do it, others will not you need to call around. It depends on their ownership and corporate philosophy. Those that will do a transmission fluid change or exchange or drain & fill certainly will have a more profitable service department!
