Sounds like you have the halogen lights as I pulled all of the plastic and even using a inspection camera I couldn't see anything resembling an adjuster. No hex or philips type of adjustment. I'll try again some other day as I got to believe something is there.
OK after taking a close look through the hole under the rubber seal I can see the tooth of the adjuster wheel. Unfortunately the phlips head screwdriver that I have seems to be to small to lock in and turn the wheel. I have a larger one but that is to large to fit into the hole and ones that fit with a larger tip are to short. So I'll have to go get another screwdriver and am asking for those who have done this what No. and length is the screwdriver . My bet is the dealer has a tool for this but I'll have to see what I can get.
I Found the adjustment access point you described and used a phillips screw driver and I was able to move the adjustment but the it doesn't move it up or down in either direction. Is there something I'm missing? Mine is a 2007 ES350 purchased in December of 2006

After reading multiple ES350 headlight adjustment questions, it seems that there's a communication gap on what actual system is in place or if there is definitely an adjustment for a specific headlight type.
My 2008 ES350 has the standard halogen headlights which needed to be raised before and after replacing the left side bulb. This was a challenge bc there is little room unless removing the battery or fuse box, plus the wire retainer clip wouldn't re-attach to the tab.
Am aware not to raise the left side too high, but both sides are too low! Is there an adjustment screw at the bottom of the hole under the fender gasket as illustrated in several photos? I don't think the car is equipped with AFS, but not sure how to determine.
I used an extra long #2 Philips which easily reaches the bottom but doesn't bite into anything no matter how it's angled. When shining a light down the fender hole under the gasket on either side, there doesn't seem to be anything?? Please advise. Thx!
The dealer, at least mine, adjusted the headlights way too high. While I don't like the headlights aimed low in front of the car what the dealer did was out line even for me.
The adjustments are easy, quick and simply, there is little to screw up, except to adjust too high or too low, which is easy to correct. Like 'jcg' said, a slight adjustment on the wall can be huge on the road, adjust a little at a time.
Responses to this headlight question have been inaccurate. I have a 2011 es350 and the low beams were aiming at the ground since I bought it used.
I had to remove the battery to access the adjustment "hole" which you will need a large size allen wrench. I believe that it was a 5/16" and about 4" long. Anyway, to raise the low beam headlight, the group of 3 adjusters are located behind the back of the head light and the "one" closest to the light. Turn "counter-clockwise" to raise. Everyone who commented on this topic said the opposite direction and the incorrect adjustment access hole for raising light. Hopefully, this helps!

I know that this is a very old thread but just wanted to let you know that you're 100% correct. I was doing what was suggested and my lights just drifted to the side did not go up or down. Only when I started to mess with the other screw that's actually visible behind the light I realized that was the up and down adjustment not the screw behind the light that everyone said it was. Also it's not counter closkwise but rather CLOCKWISE to adjust the beam upwards and there's really not much info about this anywhere. I read the manual few times and that doesn't mention which screw adjusts the up and down beam but I finally found it by messing around.
