Default brake rotor question.

hello. i am changing my girlfriends brake pads right now due to a squeaking/grinding noise. the grinding sound is only heard at the last moment of a quick stop (like a metal to metal).
so, i took off the tire (lugs were nice and tight) and depressed the caliper. the second i did, the rotor just fell and hung on to the lug bolts. ive changed allot of brake pads and have never seen anything like that happen. is that normal???
i know she had them replaced about a year and a half ago the pad wear is even and uniform.. possible wrong rotor???
advice is much appreciated!!!

The rotors on many cars are held in place by the wheel lug nuts. Once you remove the lugs, and push the pads back the rotors are free to flop around.
