It was a Denso. Nice catch. I went for another Denso via Amazon. Great price, Great shipping next day.... Unfortunately same result. (they did take it back and refund though...).

Hello to those still out there that can help. Here is ANOTHER update...

After doing everything I posted about in post # 13... My scanner / code reader came today. Like a kid on Christmas I connected the damn thing in the Pittsburgh rain, outside, with my suit pants and dress shirt. SOAKED. (I knew as soon as I went inside and played with the kids I wouldn't make it back out in the rain... )

Anyway, I scan and get the only code I have been getting for 3 weeks now: P0158 - Bank 2 Sensor 2 High Votage. So I reset that damn thing immediately... Again. (The car seems to run PERFECT until the CEL comes on, then the "fail safe" mode makes it run...well...not so perfect it seems. Can this be only in my head now? )

Difference this time, I can confirm with my new toy!! As you can see in my pictures the sensor does seem to be throwing a different voltage.
Bank 1 Sensor 2 is a constant .095 (volts???)
Bank 2 Sensor 2 shows in first pic .075 V

In next pic Bank 2 Sensor 2 shows .055 V .... With B1 S1 consistent at .095.

Pic 3 was just another pic added. I had another one for Bank 2's but it was blurry...and its still raining.

So my questions now are:
1. Can this really be a bad B2S2 sensor? This would be the 3rd one replaced in 3 weeks! And it does appear to be working right?
2. Can a loose wire somewhere cause these readings?
3. Can a pinhole leak near the sensor cause this voltage reading? (BTW, I thought P0158 is high voltage but isn't the scan pics showing it is a lower voltage? I would think air coming near the sensor would cause it to be lean and lower voltage?)

Anyway, lots of questions...sorry... that is all for now and I welcome anyone's thoughts, ideas, advice.... I know you're all smart so let it fly....

Bump if there's anyone out there that can answer my questions or review the pictures attached.

I'm still getting B2S2 code, even after 3 new sensors. No sign of wire damage. Code has been taking longer and longer to set, but still sets...

Thoughts....

Well, to everyone out there who helped or attempted to help, my problem has been solved for 3-4 weeks now.

After everything that I went through, I finally took the vehicle to Lexus and planned on getting a number of things done. Essentially, I bit the bullet and did not want the car back until it was fixed.

They ended up doing the following:
1. Replace B2S2
2. Replace B2S1
3. Performed the 90,000 maint. at my request
4. Replaced all spark plugs at my request

Car has been running GREAT since I drove it out of their lot.

(Considering I started this in November 2011, the $$ I tried to save with sensors from Amazon and buying a scanner, did not justify the PITA that this was for 3 months. I learned a LOT which is the greatest take-away, but I felt I just had to let the pros handle it at this point.)

Thanks for the update. Replacing sensors and dealing with error codes is very annoying and tests your patience. I was wondering why you decided to change your spark plugs, and what type of spark plugs did you use?
Thanks in advance,
Chris

Chris, since I purchased the vehicle used, and had the significant sensor / engine coil issues, I felt I should just replace the spark plugs while they were doing the 90,000 mile maintenance. The book says the plugs don't swap til 120,000, but I have to tell you, it made a HUGE difference in power, mileage, and smoother acceleration. I requested that the old plugs be left for me to review, and I am glad I did! 2 of the plugs showed significant burning on them, 1 of the plug gaps was very bad (wide), 1 plug gap was off center, and the other 2 appeared fine to what I would have expected on a car with 90k miles. Given what I saw, I would NEVER wait for 120k miles to change on my other Toyota/Lexus vehicles.

Also note that it is a total pain to change the plugs yourself. From all I read, it is to for the faint of heart and very involved. My dealer cost was 288.00 for ALL. It was NOT a 500+ deal, and it was the best 288 I could have spent.

On a side note, they put the old plugs I requested to see in the new red and white Toyota plug boxes. Yes, they were denso for toyota oem.

Hope this helps! Don't wait for 120k for the plugs!

My 07' RX350 just threw the same two codes. PO158 and PO606.

Bank 2 Sensor 2. I'm going to go ahead with Denso and just have Lexus do it. My local non dealer mechanic was confused by the codes and I plugged my reader in and voila....

My Lexus has 120k on it and I had them replace all spark plugs and coils last year, it needed it. The ignition coils have been a regular problem. Prior to that I swapped 4 of them. The front ones...simple...the back...HUGE PITA. In fact I tried and tried and couldnt do it. Hands are too big.

Anyhow, hopefully when I take it in there today the PO158 and PO606 will go away by swapping the Bank 2 Sensor 2. You went through.....what I don't want to. I want to get another two-three years out of this vehicle.

Great thread though....I'll just pay the $ at Lexus I suppose in this case.
