I believe you can buy new pistons from Toyota or other sources. There are rebuild kits that either has the dust caps and rings or the entire caliper without the caliper shell. May not be cheap but it's available.
When I was rebuilding my LS400 calipers I had a few pistons that were having baked on brake dusts, I just used 2000 grits sanding block carefully sand them off. Takes some times but it's doable. So I guess before you buy the pistons, might as well try to sand them off and see if you are satisfied with the results.

In the process of cleaning the calipers for G2 paint and replacing the piston boots, clips, and pins (via Toyota online site- much like the Sewell of old). I also ordered new rotors (Hart), pads (OEM Lexus and Toyota), and stainless steel lines (StopTech). This is going to be a big (and costly) project for sure. Got one caliper completely cleaned, primed, and ready for paint. Took quite a while. Lots of caked on paint. Anyway, please wish me luck... I'm gonna need it.

what are you using to remove the old paint?
Brake Cleaner?
A sand blaster sure would be nice right now!
