Default smokes when filling up the power steering..?

here's the story, this guy at my work place drives an 92 sc400 and he says that everytime he pours power steering fluid into the power steering, and whenever he starts the car it would smoke like hell...i was like WTF, how the hell do you just pour power steering fuild into the power steering and smoke like that..? so it had me thinking, maybe his head gasket or one of the piston ring is bad..but he said it only smokes when he fills the power steering up. so i'm guessing maybe he or someone accidently stick the power steering hose straight to the engine or intake manifold that's why it's probably burning it. anyone might have a clue?
