Surging at Idle (when warm only) SC400

Surging at Idle (when warm only) SC400
I hope someone might be able to help with this situation,
I have a 93' sc400 and just as of late and only after the car has been driven for a period of time does it begin to surge at idle. RPMs seem to fluctuate between 400-750 and occasionally stalling out. Also wanted to note when this issue starts happening the shifting at low speed seems to be very jarring and it really can't make up its mind what gear to be in.
Appreciate it alot for any feedback!!

Not throwing any codes when cold,than took it for a long drive on the highway last night, got it warm and starting to surge and checked for codes and nothing.
I will have a look at the vacuum system.
