When is the time to change brake pads

You mean, Lexus doesn't have some sort of position sensor on the calipers that would indicate too much travel, hence brake pad wear? Shouldn't they do that? Isn't this 2014?

In several luxury cars, including other Lexus models (LS, GS, etc) there is a brake wear sensor that breaks triggering an idiot light in the dash. This requires you sensor replacement on top of the brake job. I just picked up my GX, but I haven't seen anything in the manual that suggest that its equipped with that function.
As mentioned above, I would just check every oil change and keep an ear for that squeal around 60k.
