That looks like a tow hook location mount? I didn't need to fish the bolts through the frame, each of the frame rail mount points should have pre-threaded nuts welded inside the frame. I think your picture shows this being installed incorrectly.
The frame and hitch not lining up exactly is common. Usually. if you just got one side started, and let it hang, you can move the other side a little bit to get the holes to line up enough to get a bolt started on the other side. It takes some force. Leave all the bolts just barely started until you get all bolts in.

May have posted this in the wrong thread - I have a thread here where I was replacing the factory 12mm bolt that broke the weld nut loose in the frame. I hacked off the original bolt and installed a grade 8 nut/bolt combo in it's place by fishing through an opening in the frame rails. Picture is of the 2 12mm bolts on the passenger side near exhaust.
As for the alignment, what I did was start the 2 forward most bolts where the pintle would mount, hang the hitch, install the other 2 bolts in the pintle, and then ran into the issue with the frame rails not lining up.
It would take quite a bit of force when the bolts are off 1/2" to 3/4". I could see a situation where the alignment was off 1/8" to 1/4". This is normal?
