Update: Got the replacement sensor and installed it today. CEL hasn't come back for 23 miles so far. Before it would come back after 5-8 miles so this is looking good!
I also replaced the battery (the panasonic was the original and that would make it almost 10 years! we bought the car Oct 2006) with a Costco interstate. Easiest battery change I have ever done.
A few info for people who might get the P2195 code:
Sensor 1 bank 1 is the one by the fire wall. Lexus OEM # is 8946733130 and it is called the Right Hand air fuel sensor. My local lexus quoted 260 just for the part. Lexus of South Atlanta was 174.24. I did some hwk and discovered that the OEM Denso is the same as Denso 234-9050.
In other words Lexus part# 8946733130 = Denso 234-9050. I got the part from Rock Auto for 106. (The front/radiator side is called Left Hand air fuel lexus part#8946733090 and the Denso equivalent is 234-9041)
People always say to spray penetrating oil which I did but no one ever warns about the smoke that comes from the leftover oil burning. LOL. So be careful when you use that. I started the car and let it run for a few minutes. I heard it helps with the removal of the sensor; but be careful not to burn yourself if you warm it too much. I got the electrical connection and the bracket connection from the top (after removing the covers and the brace) I then found it easier to get the sensor from the bottom. I used a short oxygen sensor tool with a 3/8 ratchet. Remember safety first! I used 2 jackstands, chocked the rear wheels and still had two jacks supporting the car. Hardest part for me was removing the clip from the bracket. I wrecked the clip.

After doing some research I am 99% sure the bank 1 sensor 1 is the one near the firewall toward this pic:

The p2195 is saying that AF ratio fuel sensor 1 bank 1 is stuck on lean. Ill post updates when I get around to changing this. Thanks to Lexus of South Atlanta for the help in identifying this part.
