How many miles are on the motor? Have you ran a compression check? Do you have the original O2 sensors, or have they been replaced? Another possibility is a leaky injector, have you tried injector cleaner?

What he said ^. Change your O2 sensors and spark plugs, if you haven't already.

Im not to familiar with the 1992 ES300, but wouldnt there be a CEL if the emissions is bad or this pre OBDII? I had an issue with my 91 Nissan Sentra, not passing smog in Calif. and was told to replace the Cat. Converter, which I did and I passed smog. I was told if your car runs rich over a period of time, you can burn out your Cat., then you'll have passing smog issues. Good Luck.
