SoftScrub with Bleach bathroom cleanser has worked extremely well for me for more than a decade with these plastic headlamp lenses on several of our vehicles. I polish them every six months or so using this stuff on a damp 100% cotton rag, buff them dry with another 100% cotton rag, and that keeps them sparkling clean for me. Takes me only about two minutes per side and the more elbow grease utilized, the better the results.

Follow the treatment with clear uv protection. once the protection is lost, yellowing starts.

I used Mequiar's Keep Clear Headlight Coating Spray after restoring my headlights. It provides long lasting UV protection.

Sand them down and apply Clear Coat is the permanent fix.

^ This. Our other car suffered from a delamination issue common to that make & model just a few years in. We had the headlights polished & clear coated and they've been going strong for 6 or 7 years now.

When you are ready for replacement, I would find a reputable third-party manufacturer, which you can often find listed on RockAuto.com if you check their site... for example, I got a set of TYC headlamp assemblies for a 1st gen RX once, and those were great. The housings for these 3rd gens seem to be a bit more expensive, given the projector style, but certainly much more cost-effective than buying from a dealer.

A good polishing job and sealant may do the trick for a long while, though, without having to replace them. I have used Wolfgang Profi ceramic coating on headlamps on my 3rd Gen after polishing them, and the ceramic coat seems to do a good job of protecting the lenses.

Thank you for your suggestion. Of course I do not service my RX450h at Lexus dealership since they charge for everything an exuberant price. I found a local Toyota/Lexus private automotive service and the brothers service my vehicle honestly and with brand dignity. The advised me not go even in desperation to local Lexus dealership since they will charge me over $2K for replacing the sealed xenon-HID headlight housing instead to have a procedure of disassembly and clean from inside. This is why the first original owner of my 2011 RX450h decided to sell the vehicle w/ only 76K mileage for all this 13 years frustrated to live with this blemished headlights from inside w/ condense and less clear plastic. I’m 78-yr senior and thought having old but with low mileage RX450h will serve me to the rest of my driving life?!

Unfortunately RockAuto is not offering the entire sealed headlight assembly including bulbs for 2011 RX450h w/ xenon HID headlights with condense and plastic blemish from inside? There are other merchants coming online offering the headlight assembly, but not sure which one I can trust and order left/right complete sealed assembly for replacement by my local Private Toyota/Lexus automotive service garage. I do not service my vehicle at local Lexus new car dealership for obvious cost reasons.

Keep in mind that most of these plastic headlamp assemblies can be baked in an oven at a low enough temperature to allow the adhesive holding the pieces together to become soft enough to allow you to pull the pieces apart. This allows you the ability to carefully clean the headlamp assemblies' interior lenses. The assemblies are then baked again to soften up the adhesive enough to allow the pieces to be glued back together. You can find videos on YouTube showing you exactly how to do this.
