Unhappy Having some problems. Any suggestions?

I have a 1999 Lexus RX300. I am getting a rough idle when stopped and a bit of hesitation upon acceleration after coming to a stop. Also, a slight smell of exhaust coming into the cabin of the vehicle when stopped. When at 30, 40, 50, 60 mph, she runs beautiful and no smell of exhaust. Sometimes the check engine light is on, sometimes it's not, and sometimes it blinks. We have replaced all of the O2 sensors, the mass air flow sensor has been replaced, the spark plugs have been replaced, and it was taken apart to test for vacuum leaks which there are none. Has anyone experienced anything like this before and what did you do to correct it? Any suggestions are appreciated. Thank you!

If your check engine light is flashing, you have some kind of misfire happening. Either plug wires, or bad coils, if you already put plugs in it. It could also all be caused by a leak in the exhaust, which sounds like this might be the case. If you have fresh air leaking into your exhaust, its gonna throw off all your 02 sensors readings, and probably cause a bad idle. If air is leaking in, that means exhaust is also leaking out. That's probably your smell. I'd go after that exhaust all day.
