Could be bad tensioner or idler pulley. Remover belt and rotate the wheel to feel any binding or evidence that can cause it to go bad.

It didnt work, replaced the tensioner and the same problem, also when i turn the wheel also. The belt is sliding side to side when i give it gas. After putting the belt on it actually started to break. Anything else i could try. When i made the belt tight with the other pulley sliding on top it stopped squealing and sliding how can i make the belt tighter?

That's not good if it's sliding side to side. It means that there is a misalignment somewhere. Check to make sure all pully are tight and check all hardware that holds the alternator, P/S, A/C.

Check to see if your Crank pulley is still ok, at years of wear and tear the part that holds the belt will just break. I had the same problem happen twice, the last time I just bought a new one just to avoid the headaches.

Dealer part, either toyota or lexus. it's not gonna be cheap! it's within the 3-400 dollar range, I don't remember the exact amount. Or maybe you could find a good one on ebay/craigslist.
