Default need help with code 25!!

I have 95 5sp sc300..
The car is ruuning a little rough and missfire, i can hear on the back of my exhaust like putt putt, and i decided to do tune up, i did change cap, rotor, spark plug and wires, and did fuel system cleaning also, after i did evrything, it is still missfire i though i did wrong and double check agian, everything is ok.
and now i have a code 25 ( Air-Fuel Ratio Lean Malfunction )..
Can anyone help me with this problem??
Now i can feel like, i am running on 5 cylinder.
do i have to check my fuel injector or replace my fuel filter??

same light just came on I just did a search and try changing you primary o2 sensor.Im going to try to change it to see what happens!
