So how often does steering rack and pinion go out? Pretty frequently if 200k miles of not changing power steering fluid?

They are pretty reliable, and depends what you mean by 'going out'. Unless there is a true mechanical problem with a rack, there is no need to just replace it.

If there are some smaller leaks then I would replace the fluid with M1 ATF (and maybe some AT205) and see if that solves the problem. Do not forget to clean the fluid reservoir because it could limit the fluid flow and make it look like the steering rack 'went out'.

Personally, I have replaced inner and outer tie-rods. I would not call that a 'rack and pinion go out'. That is just maintenance and not that difficult to do. But it would not surprise me if the dealer just replaced that rack at that point (or recommended it).

Yup, got a PPI from a dealer and that's exactly what happened, ha.

Did more research the past two days and will buy the vehicle as seller dropped the price themselves to the point I think it's a great deal (seller's aunt bought the LS430 originally at said dealer, along with a later 460)
